Unfortunately, she was one of many who didn't survive the Jedi Purge (aka Order 66). She not only appeared in the Star Wars prequel movies (played by Orli Shoshan) but the Clone Wars animated series as well. For those who don't know or perhaps may not remember, Shaak Ti was one of the Jedi Masters who sat on the Jedi Council during the Clone Wars.
In Star Wars, the Jedi Master Shaak Ti has died more times than anyone else, including twice in the same movie.
